I understand that participation in training, rentals, lessons, camps, or other related activities at Ace of Diamonds involves inherent risks, including the risk of serious injury. I voluntarily accept these risks on behalf of the participant. In consideration of being permitted to participate, I release and hold harmless Ace of Diamonds, its owners, staff, and coaches from any and all claims arising out of ordinary negligence in connection with participation in facility activities. The participant and any adults or children accompanying the participant agrees to follow all posted facility rules and staff instructions at all times.

I authorize Ace of Diamonds staff to obtain emergency medical treatment for the participant if a parent or guardian cannot be reached.

Ace of Diamonds may photograph or record training sessions and events. I agree that I consent to using photos of the participant may be used for facility or coach promotion. This checkbox is optional.

I have read and agree to this waiver in full. I understand this form is legally binding.
